The Long Term Care Continuum - What's that about?

Long term care is about options. Options are important because people age differently and have very unique needs for support.

Adult Day Care
In a center during the day; offers less than 24 hour therapeutic services and food; resident needs transportation to and from.

Adult Family-Care Home
Full-time, family-type living arrangement in private homes for up to five aged or disabled persons who are not related to the owner. Functional criteria must be met.

Assisted Living Facilities
Facility-based; provides housing, meals, and one or more personal services for residents who are not related to the facility's owner. Functional criteria must be met.

Home Health + Nurse Registries
Provide services in private homes, including nursing and physical therapy.

Continuing Care Retirement Community
Offer different levels of care from independent living in an apartment or house to a skilled nursing facility all within the same community.

Hospice
Offered in home or in a facility; provides a coordinated program of services including pain control and counseling for residents with a terminal diagnosis.

Skilled Nursing Facilities
Facility based; offers nursing care, personal care, custodial care, and rehabilitative care to persons who are ill, recuperating after surgery, or physically weak.
