Conduct regular visits to referring hospitals to build and maintain relationships with Discharge Planners, Case Managers, Social Workers, Physicians and all other potential referral sources.
Conduct sales calls to identify new and potential referral sources. Set up luncheons, breakfast meetings and in-services at the hospitals or physician's offices for appropriate staff.
Set up tours at facilities/ALF for new referral sources.

6. Cultivate leisure time activities such as games, arts, crafts, movies, etc.

7. Develop activities which offer opportunity for physical movement such as chair exercise, bowling, active games, etc.

8. Monitor and care for all equipment and supplies, used by the activity department.

9. Ensure the full and continuing transportation coverage by managing the transportation schedule and staff.


10. Coordinate special events with marketing.

11. Help coordinate in-house special events open to families and friends (special parties, holiday meals, etc).

Qualifications- Education, Experience, and Skills:

3. Previous experience in health or social services field. Must have at least six (6) months experience in providing planned activities in a setting for the elderly, or a comparable experience. Teaching background helpful.

4. Must possess, as a minimum, a high school diploma or equivalent. A college degree is desired, but certification or experience may substitute for degree.

5. Must meet state-specific requirements.

Director of Nursing (Winter Haven) Senior Health Management, a proven leader in Skilled Nursing Facility Management, is currently conducting a search for an experienced and caring Director of Nursing to join our growing team. This is an exciting time to join SHM as we are experiencing continued growth.

The Director of Nursing is responsible for developing, organizing, evaluating and administering the patient care programs and services of the Center. This position assumes twenty four hour responsibility for the overall delivery of nursing services and ensures the implementation of all clinical policies and procedures. DON supervises all nursing staff while following all company policies and procedures.

POSITION RESPONSIBILITIES

Makes daily patient rounds with the appropriate manager(s) to note resident/patient conditions and to ensure nursing personnel are performing their work assignments in accordance with acceptable nursing standards.
Ensures center compliance with applicable local, state, federal and other regulatory agencies and corporate quality assurance standards, certifications and licensure requirements.
Leads and manages the General and Restorative Nursing Services on a 24 hour basis to ensure the delivery of high quality comprehensive patient care.
Performs physical activities including standing, lifting (up to 60 pounds unassisted), bending, stooping, pushing, pulling and twisting.
Attends and participates in inservice classes, performance improvement (“PI”) committees, and other meetings as required.

KNOWLEDGE, SKILLS, AND ABILITIES:

* Possess a current license to practice in the State as an RN.

* Previous Long Term Care experience required.

* Must have current CPR certification.
