District Program CEUS

To support FHCA Districts that are unable to meet in person due to the COVID-19 pandemic, Districts may have access to FHCA's Zoom account to host their virtual meetings, based on account availability. The calendar below indicates when meetings have been scheduled; however, not all meetings require the use of FHCA's Zoom account.

Please contact Carla Adams in the FHCA Quality Affairs Department at cadams@fhca.org or by phone at (850) 224-3907 to a) request use of FHCA's Zoom account if needed and discuss the requirements for CEUs for virtual meetings. Districts should also review the information below before submitting your meeting's CEU application.
 

Application Submission Deadline (Minimum 5 Business Days Prior)

Pursuant to Florida Administrative Code (F.A.C.) 64B-5.002 (1), continuing education providers must submit continuing education program information to CE Broker prior to the program date. To comply with this State rule, FHCA must receive the District's co-sponsorship application at least 5 business days prior to the date of the program/meeting. Applications submitted on or after the program/meeting date will not be accepted as late submissions cannot be submitted to CE Broker.

Virtual/Online District Meetings: To provide CEUs for virtual meetings, the District must provide an evaluation form to attendees and record attendance to provide to CE Broker. Districts should save evaluations electronically if possible.

Cost Per Credit Hour ($150)

There is a $150 fee per 1 CE credit hour for each program/meeting. An additional fee of $100 is required for each additional hour, up to a maximum of 2 hours. FHCA accepts all major credit cards; payment is made through the online CE Application.

*Please note, the two-hour Medical Errors course required for licensure is not an eligible program for FHCA/District CEU co-sponsorship.

Required Information

When FHCA lends its provider number to a District, the District becomes a co-sponsor of the educational offering and is responsible for providing the information required by the F.A.C. for continuing education providers. Before submitting your online application using the link below, review the application checklist here.  Note: If you are submitting for an FHCA pre-approved course, the following program information is not required (check YES to indicate the program is pre-approved).

  • Program title, date, time and meeting location address
  • Contact information for the person completing the form
  • Learner objectives (three required)
  • Speaker(s) name and contact information (name, organization and email)
  • Speaker(s) short bio (may be brief, but must include statements about the speaker's education and experience related to the topic)

Attendee Roster Requirements (due within 10 days)

Pursuant to F.A.C. 64B-5.002 (2)(a), FHCA must submit the license numbers of licensees who participated in the continuing education program within 30 days of program's completion. Please download FHCA's District Meeting Attendance Roster and use it at the meeting to document attendance and obtain license numbers.

For In-Person Meetings: Typed rosters without attendee signatures are not accepted by CE Broker.

For Virtual/Online Meetings: Provide roll call or registration roster list with license numbers.

  • May take roll call and document with District Meeting Attendance Roster form; a district president may sign the Meeting Attendance Roster form as attestation of the attendance of the listed participants.
  • In Zoom - when scheduling a Zoom meeting, request the "registration required" box be checked to record those who plan to attend the district meeting. This can also be used as a roster to confirm attendance.
  • If the meeting is recorded, the report can be downloaded in Zoom by clicking >Admin>Account Management>Reports>Meeting and click on the meeting.
