  This tool meets the requirements for a facility-based risk assessment as required in the CMS Emergency Preparedness Rule. Developed by the Big Bend Health Care Coalition and the FHCA Emergency Preparedness Council, the tool is based on the Kaiser Permanente Hazard Vulnerability Analysis (HVA) widely used by healthcare facilities of all types across the nation. The Excel file includes Instructions for completing the HVA and worksheets for several hazard types with formulas that generate risk probabilities. Note: The Emergency Preparedness Rule does not specify use of a particular risk assessment tool, only that the facility conduct a facility-based risk assessment; this HVA tool is an example of one that would be acceptable for a facility-based risk assessment. Centers must also document a community-based risk assessment (contact your local Office of Emergency Management or Healthcare Coalition for this information).
